Web Hosting Request Forms Not Working

I havnt seen this posted but i wanted to look at how the forms were constructed etc. but found that the forms arent working.

Actually, the request form is working perfectly fine. However, it's also showing the source code as well as running script in the same page.I believe vujsa's hosting is malfunctioning or he is fixing the server/script. Vujsa runs request form from his own hosting.UPDATEVujsa's form is up and running as usual. Topic closed.
