Submit Form Sending Data To Email In Table (Or Excel?) Format

Recommended Posts

Hey all, new user here. Looks like a lot of smart people here so I thought I'd ask my question here. :)I'm setting up an RMA submit form for users with problems with their devices that they purchase from my workplace. The url in question is here:http://forums.xisto.com/no_longer_exists/(I took the submit button off for now)My managers decided they wanted something more comprehensive and in a table format when the email is received by us. Something that looks like this:http://forums.xisto.com/no_longer_exists/404.png would be the best way to go about setting up forms which will send an email to us with either a good looking table, or even a .doc or .xls file that is sent from the server?I'm hoping to be able to do this without needing to access a MySQL database but if that's not possible let me know. Thanks in advance!-PCI Tech

I'm setting up an RMA submit form for users with problems with their devices that they purchase from my workplace.

My managers decided they wanted something more comprehensive and in a table format when the email is received by us. Something that looks like this:

What would be the best way to go about setting up forms which will send an email to us with either a good looking table, or even a .doc or .xls file that is sent from the server?
